At least 40 veterans died waiting for appointments at the Phoenix Veterans Affairs Health Care system, according to a report by CNN.

While the agency kept records showing appointment times for patients within two weeks it kept a “secret waiting list” of veterans -- up to 1,600 -- who waited months to see a doctor, according to the report.

Emails given to CNN showed that top management at the hospital was aware of it and that some leaders even approved of it.
